Rural Health Transformation (RHT) Western Career Coach & Navigator
General Summary:
Under the RHT Regional Lead, the RHT Career Coach & Navigator supports participants enrolled in the Rural Healthcare Transformation (RHT) grant by providing career coaching, navigation, case management, and barrier removal services to support entry, retention, and advancement in rural healthcare careers.
This position works across healthcare employers, workforce systems, and community-based organizations to ensure individuals are connected to training, support services, and employment pathways in high-demand rural healthcare occupations.
Principal Duties and Responsibilities:
- Provides individualized career coaching and navigation services to adults pursuing rural healthcare training, credentials, and employment.
- Conducts intake, assessment, and ongoing case management for participants enrolled in RHT-funded programs.
- Identifies and addresses barriers to participation, including transportation, childcare, financial needs, academic readiness, and other social determinants impacting workforce participation.
- Connects participants to support services and funding resources, including workforce programs, state and community-based assistance, and training support funds.
- Collaborates with employers, training providers, and CCSNH staff to ensure participants are successfully enrolled in healthcare training, credential programs, and work-based learning opportunities.
- Provides ongoing monitoring, coaching, and support to improve retention, completion, and employment outcomes in rural healthcare pathways.
- Conducts outreach and information sessions with community-based organizations to expand access to rural healthcare career pathways.
- Supports equitable access to healthcare careers for underrepresented populations, including low-income individuals, veterans, individuals with disabilities, rural residents, and multilingual learners.
- Maintains accurate and timely case notes, participant records, and reporting documentation in compliance with grant requirements.
- Works closely with RHT data and program staff to ensure accurate tracking of participant outcomes and performance metrics.
- Provides technical assistance and guidance to participants on navigating healthcare training systems and career pathways.
- Participates in regional meetings and collaborates with workforce and healthcare partners to strengthen the rural healthcare pipeline.
- Complies with all CCSNH, state, and federal rules and regulations.
- Performs other duties as assigned.
Qualifications:
Education: Bachelor’s degree in human services, education, social sciences, healthcare, public administration, or related field.
Experience: Two (2) years of experience in career counseling, case management, workforce development, healthcare support services, or community-based human services. OR
Education: Associate’s degree in human services, education, social sciences, healthcare, public administration, or related field.
Experience: Four (4) years of experience in career counseling, case management, workforce development, healthcare support services, or community-based human services.
License/Certifications: Valid Driver’s License or access to statewide transportation.
